پرسشنامه یکی از ابزارهای رایج تحقیق و روشی مستقیم برای کسب داده های تحقیق است.
پرسشنامه مجموعه ای از سوال ها (گویه ها) است که پاسخ دهنده با ملاحظه آنها پاسخ لازم را ارائه می‌دهد. این پاسخ، داده مورد نیاز پژوهشگر را تشکیل می‌دهد. سوال های پرسشنامه را نوعی محرک-پاسخ می‌توان محسوب کرد. از طریق سوال های پرسشنامه می‌توان دانش، علایق، نگرش و عقاید فرد را مورد ارزیابی قرار داد، به تجربیات قبلی وی پی برده و به آنچه در حال حاضر انجام می‌دهد آگاهی یافت.
پرسشنامه‌ها نسبت به سایر انواع روش‌های آمارگیری مزایایی دارند از جمله این‌که ارزان هستند، مانند آمارگیری تلفنی و زبانی، نیاز به تلاش فراوان سوال‌کننده ندارند و اغلب دارای پاسخ‌های استانداردی می‌باشند که امکان جمع‌آوری داده را ساده‌تر می‌کند.
اگر پرسشنامه یک تحقیق به خوبی تنظیم شود می تواند نیازهای آن تحقیق را مرتفع سازد. در غیر این صورت بخشی از این نیازها برطرف نشده و باعث کاهش اعتبار تحقیق و یا حتی زیر سؤال بردن آن می شود و کل اعتبار آنرا مخدوش می نماید. اثر یک پرسشنامه بر تحقیق تا آنجا پیش می رود که بسیاری از محققین بر این باورند که «موفقیت یک تحقیق در گرو طراحی یک پرسشنامه ی خوب، جامع و کامل است».
یکی از مؤلفه های مهمی که دقت ویژه ای را می طلبد، ترتیب مناسب سئوالات است. زیرا ترتیب نامناسب باعث پیامدهای نامطلوبی نظیر کاهش میزان پاسخ و پاسخ های سوگیرانه نسبت به سئوال خواهد شد. به طور کلی پرسش هایی که در آغاز پرسیده می شود نباید بر پاسخ سئوالات بعد تأثیر بگذارد. در پرسش نامه هایی که موضوع هیچ یک از سئوالات مقایسه با بقیه جذاب تر نیست، سئوالات نظرسنجی باید در ابتدا آورده شود و مشخصات و اطلاعات مربوط به پاسخگویان مانند جنس، سن، درآمد و نظایر آن در آخر آورده شود.
